Home Support

The Home Support Service provides a range of specialist support to people with complex needs with the aim of enabling them to continue living independently in their own homes throughout Hampshire. Our clients include people with severe alcohol dependency problems, other substance misuse problems, mental health problems, challenging behaviour and people who have previously been homeless.

Each individual client is provided with a personal care and support plan specifically designed to meet their needs including personal care, assistance with day to day household tasks such as cooking, cleaning and shopping as well as emotional and social support. Home Support workers are all experienced in working with the client group accompany clients to appointments, help them manage their finances, and also support them to get involved in social activities that reduce loneliness and isolation.

Accessing the Service

The service can be accessed through referral from Social Services, Mental Health Services, GP surgeries and the Hospital Discharge Team. We also accept referrals from private individuals who are seeking specialist support either for themselves of for a relative or a friend.

Once we have received a referral the Registered Manager will undertake a detailed assessment of the individual to ensure that the referral is appropriate and that their needs can be met through the service.

William’s Story
William had very complex needs and was unable to go out by himself leaving him effectively trapped in his own home. He had little social contact with anyone and felt lonely and isolated.

The Home Support Team helped William to develop his general life skills resulting in significant improvements in his personal hygiene and housekeeping. His finances, which used to be a difficult area for William, are also now under control and with the help of his support workers, William now manages a monthly budget enabling him to pay his bills, do his shopping, and still have a little left over each month.

William’s support workers have encouraged him to take trips out with them, and these trips have been a real boost to William’s confidence. Doing his shopping, something which used to be a real challenge for William, is now something he particularly enjoys, and he often stops off for a cup of coffee and a chat with his support worker at a cafe on the way home.
